When a user is permanently banned or killed, they will be given a form asking for their new username. They simply type in their desired username and their previous e-mail address will automatically be connected to it. This means they will not have to re-verify the e-mail address.
This means that you can use your old password to login to your new username.
The points from the dead account will then automatically be transferred to the new account which now means the help ticket of 'Points Dead to Alive' is now obsolete.
